I have verified that all of my information provided is correct and accurate. I understand that if any changes are needed, it is my responsibility to contact e7 Health for revisions.

I understand I will receive my results via email in 30 minutes from the time of testing.

I understand that it is my full responsibility to obtain my own results via email, based on the email address I provided when completing my patient registration. If I have not received my results via email in 30 minutes, I understand that I should notify e7 Health for further assistance.

I understand that a positive result indicates that I am currently infected with SARS-CoV-2, the virus that causes COVID-19, and can spread it to other individuals. I understand that I should take the recommended precautions to prevent transmission to others. I understand that I should contact my Primary Care Provider and/or seek medical attention if I am experiencing any bothersome symptoms, such as, but not limited to, chest pain, difficulty breathing, shortness of breath, dizziness, and fainting.

I understand and agree that these records will be disclosed and/or re-disclosed to e7 Health staff and myself. I understand that if I test positive, my results will be reported to the Southern Nevada Health District as mandated by Nevada State Law.

I understand that the payment for this test is non-refundable. I understand that if I need a request a re-test, I will be responsible for purchasing a new test.
